The Ellsworth AFB claims office is no longer responsible for accepting and/or adjudicating household goods claims. The Air Force Claims Service Center (located near Wright-Patterson AFB, Ohio) is now the focal point for this matter and is responsible for accepting and adjudicating all household goods claims. For instructions on how to file a claim with the Air Force Claims Service Center, please call 1-877-754-1212. The Claims Service Center is open 24 hours, 7 days a week to answer your questions and concerns.
Upon delivery of your household goods and noting any damaged or missing items, ensure you submit the DD Form 1840/1840R to the carrier within 70 days after your items were delivered. You may submit this form via mail or fax directly to the carrier listed in block 9 of the form. Or you can submit the form electronically via the Claims Service Center's website https://claims.jag.af.mil/. Once that is accomplished, you have nine months to file a claim with the carrier. You also still have the option to file a claim with the Air Force Claims Service Center within two years if you miss the nine month window for filing with the carrier. Please note that submitting the DD Form 1840/1840R does not constitute filing your claim but is only a preliminary step in the claim process.
You may contact the Ellsworth AFB claims office regarding this information or any other claims question at (605) 385-4901 or DSN 675-4901.

Online Legal Assistance
The Air Force JAG Corps unveiled the new Air Force Legal Assistance Website worldwide on 1 Feb 10. The website is designed for active duty and reserve component members, retirees, and dependents. The website was created to increase efficiency and track client satisfaction with the Air Force legal assistance program. Because it is a public site, clients may access the site's features from the comfort of their homes without a CAC card. Please visit the website here.
Walk-in Legal Assistance
Base legal office attorneys cannot discuss criminal law issues with clients nor provide legal advice of any sort over the phone. All personnel eligible for services must come in personally to speak with an attorney if they seek legal advice. Most matters for which personnel seek guidance, with the exception of will drafting or will executions, can be dealt with during our walk-in hours.
The Legal Office provides legal advice and assistance on personal, civil matters to military members, retirees and dependent family members. Some of the topics include, but are not limited to: wills, powers of attorney, notaries, landlord-tenant and lease issues, domestic relations issues, consumer issues and tax assistance. Personnel who cannot make it to the legal office during walk-in hours, or need a will drafted and/or executed must call the legal office at the numbers to the left.
The Ellsworth Legal Office offers free tax preparation under the Internal Revenue Service VITA program for active duty military and their families. Please call or visit the legal office to schedule an appointment. To locate any tax forms please visit the IRS website at http://www.irs.gov/.
