Palyan 28th Bomb Wing Public Affairs ELLSWORTH AIR FORCE BASE, S.D. -- The 28th Medical Group will be implementing a new system March 23 for beneficiaries not on active duty, that will electronically store their secondary health insurance information. The Electronic Department of Defense Form 2569 is expected to reduce the amount of time it takes for patients to check in. Patients are required to register and complete DDForm 2569 annually. "When patients come in for an appointment, they will no longer have to complete a paper form and will not have to carry a Third Party Collection card," said Master Sgt. Crystal Trevino, 28th Medical Support Squadron, Medical Resource Management flight chief. "We still recommend arriving 20 minutes prior to appointments because this is a new process." Trevino added that patients should have their other health insurance card with them when they arrive at the clinic to register in the new system and expedite gathering information.
