ID card renewal for dependents

ELLSWORTH AIR FORCE BASE, S.D. -- A sponsor may apply for renewal of a dependents' identification 10 to 20 days prior to expiration of the ID, per AFI 36-3026, paragraph 15.4.

The sponsor must be present to sign the DD Form 1172, or may sign the DD Form 1172 prior to the dependant coming in. A general Power of Attorney is acceptable for signing the sponsor's signature should the sponsor be unavailable to sign. A copy of the sponsor's deployment orders is also acceptable; the ID card will then be issued to the end of the sponsor's deployment only.

Sponsor must sign in Block 90 on DD Form 1172.
