DEERS requires documentation for ID card reissue

ELLSWORTH AIR FORCE BASE, S.D. -- The 28th Force Support Squadron Military Personnel Section Defense Enrollment Eligibility Reporting System/Identification Card station requires documentation to reissue lost or stolen common access cards, due to a DEERS software upgrade, Dec. 3.

All Airmen, civilian employees and contractors who have a lost or stolen CAC are required to request a reissue memorandum from their first sergeant. In addition, they are also required to specify a reason for reissue and must obtain first sergeant or commander endorsement for their request.
